He died in the bathroom of his Manhattan apartment building. He was found by playwright Dan Katz and a friend after he failed to pick his children up from Mimi O'Donnell (the mother). She called Hoffman's friend, Katz, he and a friend went to his apartment and found him in the bathroom with a syringe in his arm. There was no note as Joe said. It appears to be an accidental overdose, not an intentional one.
